  • Line 2. Army serial number, Tetanus shot year, blood type.
  • Line 3. Name of next of kin.
  • Line 4  Street address of next of kin.
  • Line 5. City & State of next of kin and Religion preference.

If you require the later war style which only used 3 lines of information please see the other tags below. Please note the reason these tags are more expensive than the later war style it is because they are all manually stamped as there is more information to stamp it takes longer to make them.
